The programs emphasize a structured, supportive
environment for individuals to learn daily living
skills such as cooking, medication management,
budgeting and personal care. Individuals are
integrated into the community by involving them
in off-site day programming and community activities
at a pace commensurate with their function and
coping abilities. Staff provides intensive training
and assistance. As individuals increase their
independence and acquire skills, they may transition
to a less structured setting.
Admission criteria:
Individuals must have a primary psychiatric diagnosis, be at least
18 years of age, and have an affiliation with an outpatient mental
health treatment provider.

Case Management Services

Case Management serves young adults and people who are dealing
with both mental illness and chemical addiction (MICA). This program
facilitates admission into Unity House's community residences
program. Young adults with mental illness and chemical addiction
can receive service in independent community settings as well.
Case Management
Services works with both young adults and adults with mental illness
and chemical abuse. Staff meet with people to assess their service
needs and provide case management services including making referrals
and coordinating care. When the individual requires Unity House
Community Residence Services, the Case Manager continues to work
with him/her in the community. Individuals may remain with family
or an independent apartment if they choose. Case Managers provide
direct service 1-3 times per week, based on individual need. The
level of service is diverse and intensive; it may include independent
living skills training, obtaining and maintaining housing, utilities,
employment, coordinating with providers, and connecting people
to community resources. Services continue on a time-limited basis
until the services are no longer needed.
Admission criteria:
Young adult case management - Individuals must be 17 - 35 years
of age and have a primary psychiatric diagnosis. Priority is granted
to Rensselaer County residents.
MICA Case Management - Individuals must be at least 18 years of
age and have a primary psychiatric and substance abuse diagnosis.
Individuals must also be affiliated with an outpatient mental
health
treatment provider.

Situational Crisis

The Situational Crisis Bed is an alternative to inpatient hospitalization
for people experiencing acute symptoms of their mental illness.
It can be used to prevent an inpatient admission, or to shorten
the length of an inpatient stay for a person returning to the
community. Services include crisis management, medication supervision
and coordinated treatment planning within this 13-bed 24-hour
supervised group home. The length of stay is one to seven days.

MICA Residence

This program offers a twelve-bed supervised community residence
for adult individuals with co-existing mental health and chemical
dependency diagnosis. This residences primary goal is to
improve clients functional abilities in the areas of mental
health and chemical dependency and to enable movement towards
independent living and reintegration into the community.
Admission criteria:
Individuals must be eighteen years of age or older, have a primary
mental health diagnosis and co-existing substance abuse/dependence
diagnosis, have demonstrated an inability to function well in
a less supervised setting (particularly related to chemical abuse),
are currently, or willing to participate in clinical mental health/chemical
dependency treatment, are not in immediate need of detoxification
services and are
medically able to participate fully in the program. In-county
referrals will take precedence over out-of-county referrals.
