Housing Support Services provides permanent and transitional housing services for adults with mental illness, people with HIV/ AIDS, and victims of domestic violence. Services include securing housing, home visits, case management, advocacy, and crisis intervention.  



Transitional Apartment Services

Transitional Apartment Services provides safe, secure furnished apartments for adults with mental illness. Services are provided through home visits up to 7 days a week, based upon individual needs. Rehabilitative services are provided, as well as individualized service plans, apartment management, daily living skills training, case management, medication management, entitlements assistance and coordination of services with other service providers. This program is regulated by the Office of Mental Health and the
program fee is based upon income.

Admission Criteria:
Primary psychiatric diagnosis, affiliation with an outpatient mental health treatment provider, 18 years of age, resident of Rensselaer County.

Supported Housing for Mental Illness

Supported Housing for Victims of Domestic Violence housing program provides comprehensive case management services including affordable housing and assistance in securing and maintaining safe, permanent housing, home visits, life skills training, advocacy, referrals, legal services, support groups, safety planning and individual counseling. Services are client-centered and tailored to meet individual needs. Rent stipends provided based on consumer income. No program fee.

Admission criteria:
Victim of domestic violence, homeless, mental health diagnosis and/or substance abuse, income eligible.

Supported Housing for People Living with HIV  This program provides comprehensive case management services including home visits, individualized service plans, transportation to medical/treatment providers, advocacy and referrals. This program also provides assistance in securing and maintaining safe, permanent housing. Rent stipend is provided based on consumer income. Services are client centered and tailored to meet individual needs. No program fee.

Admission Criteria:
Diagnosis of HIV or living with AIDS, homeless or at risk of being homeless, ability to self-preserve.
