Unity House to provide HCBS

HCBS stands for Home and Community Based Services

Unity House can provide long-term care services and supports in the home or community. These services can be:

  • Education Support Services
  • Empowerment Services-Peer Supports
  • Family Support and Training
  • Habilitation
  • Intensive Supported Employment (ISE)
  • Non-Medical Transportation
  • Ongoing Supported Employment
  • Prevocational Services
  • Psychosocial Rehabilitation (PSR)
  • Short-Term Crisis Respite
  • Transitional Employment

Unity House is a participating provider with all the major Capital District managed care organizations. We have been providing HCBS services to consumers since October 2016.

Our HCBS program builds on our long history of providing behavioral health services, supportive housing, supportive employment, and crisis respite to Capital District residents facing multiple health and economic challenges.

Education Support Services: Unity House counselors assist consumers to define educational goals and develop strategies and supports to achieve them.

Empowerment Services-Peer Supports: Unity House’s OMH Certified Peer Specialists foster trust and inspire hope.

Family Support and Training: Our trauma-informed counselors help individuals to engage family members in their recovery.

Habilitation: Our experienced counselors provide person-centered, strengths-based training in daily living skills.

Intensive Supported Employment (ISE): Employment Specialists coach consumers in the skills needed to find and keep meaningful jobs.

Non-Medical Transportation: Unity House provides consumers access to non-medical transportation, critical to recovery and community reintegration.

Ongoing Supported Employment: Employment Specialists provide any and all reasonable supports and interventions that an individual may need to keep their job.

Prevocational Services: Employment Counselors help consumers develop “soft skills” that may ultimately contribute to employability in integrated and competitive community work settings

Psychosocial Rehabilitation (PSR): Our counselors coach participants in behaviors and skills to maximize independent self-management and community integration.

Short-Term Crisis Respite: Unity House provides 24-hour hospital diversion or step-down behavioral health crisis services.

Transitional Employment: Our Employment Specialists provide time-limited supports to strengthen work skills leading to long-term employment.
